Commonly Used Office Forms

To save time, the following forms may be printed out and completed prior to your visit. We are transitioning the forms into Adobe Acrobat files, in order to preserve formatting during the printing process. Most updated browsers will automatically open these files for you.

If you do not have a viewer for Adobe Acrobat files (.pdf), you may download the free Adobe Acrobat Reader from http://www.adobe.com/products/acrobat/readermain.html.

If you do not have a viewer for .doc files, you may download the Word 97/2000 Viewer from the Microsoft downloads site (www.microsoft.com/downloads) Click on "Office and Home Applications" and type "viewer" in the optional "keyword" dialog box, then hit "search".

Also, please be aware that we have a joke exchange program in the office. Please bring in a funny joke whenever you come in. Unless we are infused with new humor on a regular basis, you may get stuck hearing the same joke over and over again from Dr. Pearson... (Please have mercy on us all!)
